Description Salary £27,269 - £31,067 per annum | 29.6 hours per week | Part time, Permanent | Short Term Support Service | Morecambe bay 2 positions avaliable Pay Award Pending Job Purpose: We are seeking a dedicated and compassionate Social Care Support Officer to join our Short Term Support Service team in Morecambe Bay. The successful candidate will provide strength-based, outcome-focused assessments and support to adults across community and hospital settings, promoting independence and recovery. Key Responsibilities: Collaborate with professionals and agencies to deliver strengths-based, outcome-focused service plans. Conduct assessments and reviews of social care support needs, working in partnership with individuals, their carers, and other agencies. Facilitate safe and timely discharges from hospital, supporting individuals' choices and independence. Promote the creative use of personal budgets to meet the needs of those we support. Manage a caseload effectively, maintaining accurate records using electronic systems. Participate in continuous professional development and mandatory training. Maintain emotional resilience and respond to urgent situations as part of a duty system. Essential Qualifications: GCSE Grade ‘C’ or equivalent in four subjects, including Maths and English. Basic qualification in Social Care (desirable). Experience: Previous experience in an organisation providing care or health services. Experience in assessing individuals' needs for services. Skills and Abilities: Effective communication and teamwork skills. IT literacy and experience with record-keeping systems. Ability to manage budgets and resources. Knowledge of diverse cultures and commitment to equality and diversity. Ability to liaise with other agencies and coordinate services. Other Requirements: Commitment to health and safety.
