Main duties of the job Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, mental health, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
