An exciting opportunity has arisen for a Sister / Charge Nurse in Community Nursing, Carmarthenshire.
The successful candidate will be based within the Intermediate Care Multi-Disciplinary Team (ICMDT) at Eastgate offices in Llanelli and in Priory Day Hospital at Glangwilli Hospital.
Main duties of the job
The Intermediate Care Team is responsible for the delivery of a high standard of nursing care to patients and clients in their own homes, residential/nursing homes, and other care settings in Carmarthenshire.
The successful candidate will be responsible for the shared management of a team of band 2, 3, 4 and 5 nurses, and will be responsible for the supervision, management, and teaching of junior staff.
In addition to this, they will provide clinical advice to the Team and other Professional agencies, acting as a resource for evidence-based care.
The successful candidate will provide clinical leadership to the Intermediate Care Team in order to meet the needs of the caseload and local population and must be an experienced clinician. You will be responsible for the daily triage of Patients in a Health Crisis and be responsible for the assessment of accepted patients onto the caseload. As part of this assessment process, you will design a care package and set goals for a team of Crisis Response, Health Care Support Workers.
The ability to travel between sites in a timely manner is essential for this post.
