About us:
Ascot House Intermediate Care Unit supports patients with therapy rehab goals within a 24-hour setting. We provide support with personal care, medications, and therapy plans for people who have experienced recent illness or injury. Our focus is to support our patients to regain their confidence and independence. Patients are given the time and space they need to progress with their goals to enable them to return home.
The team consists of 68 Support Workers, Assistant Practitioners, 4 Senior Support Workers, and 4 Deputy Managers plus the Registered Manager. We work as a multi-disciplinary team alongside Nurses, Doctors, Physiotherapists, Occupational Therapists, and Social Workers, providing a holistic assessment of service users' needs.
About you:
Your style and approach will reflect your values, and you will be passionate about what you do, always striving to improve things, valuing and respecting others. You will take pride in your work, getting things right the first time, and learning from when things do go wrong. You will enjoy taking personal responsibility to resolve issues and change what you do to make a difference.
You will have good communication skills with both service users and other professionals.
About the role:
Support Workers provide support to patients who are undertaking a period of rehab or in the assessment unit, helping build a picture of a person’s needs in anticipation of discharge. The role requires you to be part of an integrated team working alongside our health colleagues. You will provide support with personal care. Good recording skills are essential as the records you make form part of a person’s discharge plan. We help people regain confidence with mobility and support them to lift their mood after sometimes a long period in hospital. The Support Worker strives towards promoting and maximizing the independence of service users and empowering individuals to make informed choices whilst living their lives to the full within the community.
