Aftercare Well-being Recovery Coordinator
Fixed Term contract until March 2025 with the possibility of extension
Monday to Friday 9am until 5pm, 37hrs, Based at PCRS & Aftercare Lewisham, 55 Dartmouth Road in Forest Hill.
We are currently looking for an experienced Aftercare Well-being Recovery Coordinator to work at Primary Care Recovery Service and Aftercare in Forest Hill, Lewisham. Knowledge around substance misuse and experience of working with service users on ATR and DRR court orders would be an advantage.
You would work as part of a team of Aftercare Recovery Coordinators, Recovery Coordinators, Counsellor, IPS Coordinator, the clinical team and managers.
The successful Aftercare Well-being Coordinator will be holding a caseload of adult service users who have either completed treatment and/or recently were released from prison and/or are involved in CJS (Criminal Justice System); therefore, working closely with Lewisham Probation service is essential for this role.
Aftercare Well-being service users are abstinent from their primary drug of choice but actively using their secondary drug of choice (not in a problematic way).
You would be required to complete triage and comprehensive assessments as well as Risk Assessments, Care Plans, TOPs, and SMART goals. You will be engaging service users in one-to-one psychosocial key working sessions around relapse prevention and supporting them in addressing their use of secondary drug of choice, i.e., alcohol, cannabis, cocaine, etc., as well as signposting them to other services and group activities. Your role is to support service users in successfully reaching their treatment goals.
You will be required to work closely with other professionals and treatment providers within the borough of Lewisham as part of a multi-agency working.
