We are looking for a Team Manager for the Sudbury Adult Social Care Team. This is a key role within the organisation, the post holder will lead and manage the Adult Social Care practitioners within the Sudbury Integrated Neighbourhood Team (INT), this will ensure that the highest standards of health, social care and reablement continues to be delivered within the locality. As previously indicated, the main purpose of the role is to directly manage the Adult Social Care practitioners within the INT and contribute to the effective leadership of the team. They will also support the Locality Lead in delivering effective integration between health and social care through collaboration with stakeholders and partners across the system to deliver better outcomes for the people of Suffolk. Working alongside all key partners, will be an important part of the role, ensuring the health and social care systems are joined up and accessible.
