Vision and values The aim of the NLPCN is to offer high quality patient-centred healthcare which is committed to bringing together GP practices so that they can work more effectively with others. Our ambition is to reduce inequalities in healthcare and improve the quality of health and wellbeing throughout North Lewisham through increased access to high quality, integrated health and social care. As a PCN, NLPCN operates as part of the local accountable integrated care system, working and collaborating with GP practices, a wide range of healthcare professionals, social services professionals as well as local community groups. This enables us to not only focus on direct health services but also on the social and other issues that impact on health, thereby enhancing the health and wellbeing of patients and the community. The Role We are looking to recruit to the post of Advanced Clinical Practitioner Nurse, to work within our Primary Care Network multidisciplinary healthcare team. Main Role Responsibilities The following are the core responsibilities. However, there may, on occasion, be a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels. Clinical Care Delivery Provide a first point of contact within the PCN/practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision making, to establish a diagnosis and management plan Make professionally autonomous decisions for which you are accountable Instigate necessary invasive and non-invasive diagnostic tests or investigation and interpret findings/reports To prescribe safe, effective and appropriate medication as defined by current legislative framework Provide safe, evidence-based, cost effective, individualised patient care Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person Refer patients directly to other services within or outside the PCN, and agencies as appropriate Participate in developing and delivering specialist primary care services such as long-term disease management (diabetes, asthma, chronic obstructive pulmonary disease, coronary heart disease and hypertension). Work with nursing, medical and health care assistant colleagues across the PCN to ensure that National Service Frameworks (e.g. Coronary Heart Disease/Older People/Diabetes/Mental Health) are being delivered Work with the PCN multi-disciplinary teams as well as those within the PCN member GP practices and across the wider CIC, to promote integrated and seamless pathways of care Contribute to the PCN achieving its quality targets to sustain the high standards of patient care and service delivery Participate in identification of community health needs and develop patient/family-centred strategies to address them in liaison with appropriate community health professionals and agencies Be aware of and contribute to, planning and delivery of practice-based commissioning This is not an exhaustive list and may be amended dependent on staff competency and service need ANPs cannot provide care to pregnant women if this involves assessment of the pregnancy, unless they are also a practicing midwife meeting the NMC requirements of registration. Professional role Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review Maintain their professional registration Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained Work within the latest NMC Code of Professional Conduct Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation Pro-actively promote the role of the Advanced Nurse Practitioner within the PCN Community ward and externally to key stakeholders and agencies Participate in multi-disciplinary protocol and patient group directions development Identify appropriate opportunities to delegate both clinical and administrative tasks to more junior staff Help the PCN operate in a cost effective manner Identify and manage nursing care risks on a continuing basis Participate in PCN meetings Participate in audits and inspections as appropriate Work closely with the clinical leads and administrative managers in the setting up and/or improving of systems for monitoring/measuring performance against the indicators for the domains and areas outlined in the current IIF Work with the PCN member GP practices to assist them meet Clinical Governance and Quality Indicator targets Ensure that all PCN policies are fully implemented Work collaboratively with colleagues within and external to the PCN Participate in audits and inspections as appropriate Ensure that all PCN policies are fully implemented
