The Scarborough based GI Cancer Clinical Nurse Specialist (CNS) team currently has a vacancy for a full time Band 6 CNS. You will join a dynamic and welcoming team of 15 staff members.
Working in close collaboration within the team, you will also build firm relationships with the larger MDT including surgical, oncology, pathologist, and radiologist consultants. You will be supported and encouraged to broaden your spectrum of skills and knowledge of GI cancers and cancer pathways as you will be expected to act as a key worker for a defined caseload of GI cancer patients.
Key Responsibilities:
1. Developing the service, identifying, and looking to improve ways of working through leading service improvement projects.
2. Supporting team members with learning and development and carrying out 1:1s and appraisals for B4 members of the team.
3. Building relationships with other specialties locally and nationally, attending GI educational events.
4. Participating in the promotion of the GI team at Cancer Awareness events.
5. Working cross site as required to ensure the service is covered - at York Hospital.
6. Providing support for patients and their families on their journey through complex cancer pathways.
7. Attending and participating in MDT meetings, advocating for your patients, and ensuring next steps and outcomes are clear.
8. Undertaking Nurse led clinics and working autonomously to provide information on GI Cancer Diagnosis to patients and their families.
9. Translating information into clear language to ensure patients are fully informed and aware of the next steps on their cancer pathway.
10. Utilising specialist GI knowledge to support your patients’ holistic needs, considering their emotional, social, and psychological alongside any physical requirements.
11. Undertaking regular reviews of patients on the caseload, providing personalised stratified care, carrying out informed assessments, and writing care plans.
12. Referring patients to relevant specialties within the NHS and external providers for additional support.
13. Carrying out end of treatment conversations with patients and arranging ongoing surveillance including requesting CTs, MRIs, bloods, and other investigations based on patient assessment and need.
