To Develop and maintain a professional portfolio and maintain clinical study requirements in compliance with NMC recommendations and provide evidence of revalidation. To critically reflect on ones own performance, for example, through the use of clinical supervision and the Trusts appraisal system. In conjunction with the North Middlesex University Trust & North London Cancer Network to work towards the recommendations of the National Institute for Clinical Excellence Improving Supportive and Palliative Care for Adults with Cancer within the Trust. To participate in working groups related to Palliative Care & End of Life and presenting at local and national leve Expert Clinical Practitioner To receive direct referrals and manage own caseload autonomously, without supervision. Taking a lead on more complex cases and working with the Multidisciplinary team. Ensure that people affected by cancer are aware that they are interacting with a Macmillan Professional and know about the full range of resources and services available through Macmillan To act as patients advocate and be present at Best interest meetings. To maintain the highest standards of care thorough assessment, planning, implementation & evaluation of the physical, psycho-social, spiritual and cultural components of all patients and their carers. To use specialist knowledge of symptom control to inform medical teams of the appropriate prescribing of medication to control symptoms. As a non-medical prescriber, prescribe medications for symptom control within your scope of practice. To communicate information and advice given, clearly on EPR systems and/or verbally for Multi-Disciplinary Team Colleagues. To empower the staff within wards and clinical areas who are completing the delivery of care to palliative or End of Life Care patients by working with the multidisciplinary team. Leader To lead by example and role modelling, mentorship. To support, enable, motivate and develop nursing staff within the Palliative Care team and in the wider aspect of the Trust including the community division To encourage innovation and develop new approaches to the delivery of patient focused care within palliative medicine. To support and guide clinical staff on the implementation of the online Trust iPELC documentation. After several years of ever closer working, North Mid and Royal Free London are planning to come together as one organisation to enable them to go further and faster in improving services for patients and the health of the local community than they can achieve separately. The merger of North Mid into the RFL group is currently expected to take place on 1 January 2025, subject to NHS England and Secretary of State approval.
