A fantastic opportunity has arisen in our Integrated Discharge Team as we are looking for a Team Manager who will provide leadership and management to a range of professionals supporting vulnerable adults being discharged from hospital and the progression of the Transfer of Care Hub (TOCH). We are looking for an individual who thrives in a fast paced and performance driven environment, that can readily adapt to new challenges on a regular basis. The role is being recruited to permanently to support our operational activity.
IDT cover a range of areas such as intermediate care and Athorpe (non-acute bed bases) as well as community reviews and supporting same day discharges from the Acute Trust (Rotherham Hospital). As team manager you will be supporting staff across all bases including Woodside where we work collaboratively with health partners in the Transfer of Care Hub.
The successful post holder will lead a team of social workers and other vocationally qualified staff to provide a service to adults with learning disabilities, carers, older people, physical disabilities, mental ill health and / or sensory impairment.
As the Team Manager and decision maker it would be your role to ensure services and support is provided in accordance with the legal frameworks governing Adult Social Care and in line with principles of Rotherham’s Adult social Care Pathway, which aims to support people to live as full and independent a life as possible in the community.
Team managers are expected to co-ordinate caseloads/ case allocations of staff, lead and/or contribute to specific project work, whilst ensuring that work is regularly reviewed, monitored, recorded and evaluated in order to maintain high quality services.
The post holder is responsible for workforce development of the reporting team, including identifying and required upskilling, providing effective feedback to staff whilst monitoring and ensuring standards are being adhered to. Team managers are required to plan for annual leave, identifying delegated authority to ensure the continuation of a high-quality service.
