Dorset HealthCare University NHS Foundation Trust
Are you an innovative, creative, motivated caring professional who is looking for a new challenge? We have an exciting opportunity to join our well-established Weymouth and Portland Community Mental Health Team.
As a Mental Health Practitioner, you will act as a Care Coordinator for a dedicated caseload of patients suffering from Serious Mental Illness, providing person-centred, evidence-based, targeted interventions to aid recovery. You will have a working knowledge of current bio-psycho-social models of care and understand how to implement such interventions with your patients to promote optimum health and well-being. You will be provided with regular clinical and peer supervision.
The team works collaboratively, and you will have the support and opportunity to develop your own skills in evidence-based therapeutic interventions in psychological therapies as well as advanced nursing.
The Mental Health Practitioner role is a Full-Time permanent position, covering the Weymouth and Portland area. A valid driving licence and access to a car are required, as you will be reviewing and monitoring patients in their homes and other community settings. Opportunities for flexible working are available as long as the service needs are met.
This is a fixed-term role, and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
Main duties of the job
You will work closely within our Integrated Multi-Disciplinary Team, which includes Consultant Psychiatrists, Mental Health Nurses, an Occupational Therapist, Social Workers, an Advanced Nurse Practitioner, Support Time Recovery Workers, a Physical Health Worker, and our Psychology Team. There will be a good deal of work with multiple professionals across a wide range of services.
You will have the opportunity to act as a mentor, assist with induction, and provide in-service training for staff when necessary. As a Mental Health Practitioner, you will maintain and develop effective links and sound communication with the Inpatient and Crisis Service, particularly regarding the aftercare of discharged patients and ensuring good communication with relatives and other visitors.
