Main duties of the job
As the Head of Community Services, you will lead, manage, and develop all Hospice Community Services under the supervision of the Director of Clinical Services. Your role will be pivotal in ensuring the delivery of high-quality, patient-centred care that aligns with the Hospice Strategy and the needs of our local communities.
You will provide strategic leadership and operational management for a range of services, including:
* Community Clinical Nurse Specialists (CNS)
* Fast Track Home Care Domiciliary Service
* New Community Outreach Initiatives
Job responsibilities
Role: Head of Community Services
Hours: Full time, 37.5 hours per week
Salary: £53,754 per annum
Key Responsibilities:
* Drive the development and implementation of community services that meet the needs of patients, families, and carers, in line with national and local healthcare policy.
* Provide management and leadership to a multidisciplinary team, fostering a culture of collaboration, innovation, and excellence.
* Offer specialist palliative care advice to internal and external stakeholders, including professional colleagues, patients, and families.
* Build and maintain strong relationships across the Hospice and with external partners, ensuring an integrated approach to care delivery.
* Monitor and evaluate service performance, identifying opportunities for improvement and ensuring compliance with clinical standards and regulatory requirements.
About You:
To succeed in this role, you will have:
* A relevant clinical qualification (e.g., nursing, allied health professional) with specialist knowledge of palliative care.
* Proven experience in leading and managing multidisciplinary teams within a healthcare setting.
* Strong knowledge of community healthcare services and policy frameworks.
* Excellent communication, organisational, and decision-making skills.
* A commitment to patient-centred care and the values of hospice care.

Qualifications Skills Knowledge and Experience
* Registered AHP, Nurse or Social Worker
* Masters degree in an appropriate area or relevant demonstrable experience
* Degree in relevant area
* Advanced communication skills course, Maguire or similar
* Teaching qualification, PGCE, 998 or similar
* Non-Medical Prescriber
* Designated Prescribing Practitioner (DPP)
* Advanced Clinical Assessment Course
* Evidence of further study in palliative care or frailty
