Job Overview
An exciting opportunity has arisen to join the Initial Response Service based at Hopewood Park. The Initial Response Service is the single point of access to mental health services in the South Locality. The service works closely with our Community Treatment Team colleagues and we are seeking to recruit a Band 6 Community Practitioner.
The post holder will be responsible for triaging all referrals into services to determine the most appropriate pathway and signpost accordingly. You will need to be flexible in your approach to respond to the needs of the service and the demands of incoming referrals and queries from both professionals, Service users, and Carers.
We are looking for enthusiastic individuals who have experience working with service users with complex needs and are able to manage risk and act accordingly in response to safeguarding concerns. Previous experience of working within the Community Treatment Team would be advantageous but not essential. The ideal candidate will be proactive and dynamic, with enhanced clinical skills to support the robust triage process.
The post covers predominantly the hours of Monday to Friday 9-5 but you may be asked to work outside of these hours to meet service needs. Flexible working can be accommodated over a 7-day week if required.
Main Duties of the Job
1. To be responsible for the Assessment and Management of casework of service users who have substantial and complex needs.
2. Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies, and the public.
3. In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.
4. To consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate.
5. To deliver evidenced based Clinical and Psychologically based Interventions.
6. The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
7. Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.
8. Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.
9. To provide compassionate care that is based on empathy, kindness, respect, and dignity.
10. To incorporate a client-centred philosophy into nursing practice and client-centred collaborative care.
11. Work flexibly to meet the needs of Service Users across pathways which may regularly include working extended and flexible hours over 7 days a week.
12. Demonstrate flexibility in the role.
Person Specification
Knowledge
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
* Evidence of therapeutic engagement.
* Interest in Crisis Management.
* Ability to work independently.
Skills
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk Assessment and Risk Management/Safety Planning skills.
* Demonstrate an understanding of effective & appropriate treatment options including a range of evidence-based clinical approaches as identified in N.I.C.E. Guidance.
* Ability to work in a team and lone working.
* Clinical Leadership.
* Ability to adapt in stressful situations.
Experience
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.

* 12 to 24 months band 5 qualified nurse experience dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
* Experience of working within a community team.
* Experience of working with Children and young people or an interest to work with Children and Young People.
