Main duties of the job Provide high-quality, patient-centered care to individuals with a range of medical conditions. Work effectively as part of a multidisciplinary team, liaising with doctors, nurses, and other healthcare professionals to ensure optimal patient outcomes. Manage a variety of cases, with a focus on rapid assessment, care, and discharge planning, ensuring that all patients are ready for a 72-hour discharge. Monitor patients' conditions, administer medications, and provide treatments as per clinical guidelines. Contribute to patient education and support during their short hospital stay, ensuring a smooth transition to discharge. Maintain accurate and up-to-date patient records in accordance with Trust policies.
