Job Overview To assess, implement and monitor the care needs of patients safely within their own homes, using technology to view observations and conducting telehealth consultations without direct supervision. To work alongside the multi-disciplinary team regularly to provide the holistic care to patients on the virtual ward. The post holder will manage their workload and escalate any concerns to their senior nurse and/or consultant on call. The post holder should at all times act in such a manner as to justify public trust and confidence to safeguard the interest of individual patients Main duties of the job Carry out telephone assessments routinely to assess and monitor patients with acute/long term conditions To document all communication and changes to patient’s care plan accurately and within trust guidelines To liaise with consultant for multi-disciplinary team meetings daily and implement investigations & interventions To communicate relevant changes promptly and effectively in patient status to the multidisciplinary team To practice as a professional nurse, acting with confidence, authority and expertise To be accountable for the assessment of care needs: the development, delivery and evaluation of nursing care for a named group of patients To act as the patient’s advocate, ensuring a caring environment – both physical and psychological To maintain accurate and legible records of care To liaise with other health care workers within the hospital and community to promote health, welfare and care of patients To lead a team approach to patient care with emphasis on clinical development To take a lead role in research and research-based practice To ensure that the Trust’s Health and Safety Policies are understood and observed and that procedures are followed.
