We provide assessment, treatment, and support for service users of any age with a diagnosis of dementia or people over the age of 65 with a mental health condition within an 8 week time frame.
The team provides specialist assessment, therapy & treatment, working to an individually tailored care plan to adopt a flexible, patient-centred approach enabling the older person with mental health difficulties to engage in services available to them and to lead as independent a life as possible.
Delivered in people’s own homes within the community, the service makes every effort to prevent unnecessary admission to hospital and facilitates the opportunity for people to return home following a hospital admission or period in residential care.
There will be an expectation that the team will work closely with current Community Mental Health Teams (CMHT) in the locality and with the CRHT’s.

This is a leadership role and you will be expected to develop specialist skills in working with patients with high levels of acuity and increasing complex mental health conditions and/or diagnosis of dementia. You will also be expected to support students and more junior staff within the team through clinical supervision and mentorship, and you may be asked to line manage some staff.
We are seeking enthusiastic team players, to work within a dynamic multi-disciplinary health team, have a sound knowledge of risk assessment and be able to demonstrate sound evidence-based practice. You will have excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies.
The health and wellbeing of our colleagues is a top priority. We invest significantly in this through our in-house occupational health and staff counselling services, supported by a dedicated Health and Wellbeing team.
