Leaving Care Personal Advisor Officer Wakefield Umbrella Pay- £17.76 per hour 6 months initially Monday to Friday, 9am – 5pm, 37 hrs/week – Hybrid Direct working with children and young people preparing to leave care or have left care Prepare, coordinate and review Pathway Plans and Risk Assessments Carry out actions as outlined in the Pathway Plan Enable and support care leavers to secure and maintain accommodation appropriate to their needs Liaise and develop working relationships with partner agencies and colleagues in accommodation Provide practical housing related support including budgeting, benefit claims, maintaining utility payments Attend and participate in Looked After Children reviews/ CAF/ CIN and other meetings which concern the children and young people Keep and maintain accurate records of work Take part in the duty rota, responding to children and young people who may be in crisis and taking enquiries from out of the area young people not known to the service Requirements UK Driving Licence, access to a vehicle and business insurance Knowledge and understanding of the difficulties young people face in making the transition from adolescence to adulthood Knowledge and understanding of the Children (Leaving Care) Act 2000 and other related legislation Knowledge of Safeguarding issues Excellent written and verbal communication skills Enhanced Child and Adult DBS registered to the update service Recruitment is done in line with safe recruitment practices. 4Recruitment Services in an equal opportunities employer.
