The ISCH staff will provide Individual Support to Massachusetts Waiver Contracted Individuals – regular or intermittent services designed to develop, maintain, and/or maximize the participant’s independent functioning in self-care, physical and emotional growth, socialization, communication, and vocational skills, to achieve objectives of improved health and welfare, and to support the ability of the participant to establish and maintain a residence and live in the community.
Responsibilities:
* To empower individuals to make informed decisions about their education, health, and care provision to achieve better outcomes.
* To assist in developing care plans and information management.
* To promote partnership working between individuals and the community.
* To collaborate closely with the interdisciplinary team involved in an individual’s care.
* To provide resources to help individuals maintain safety and independence in the community.
* To assist with training and supervising caregivers and PCAs.
* To undertake home visits and document activities.
* To carry out clerical and administrative duties as required.
* To provide necessary information about health status and experience.
* To accept only assignments for which qualified.
* To attend agency orientation, in-service education programs, and staff meetings.
* To practice safety and competency within the job description.
* To communicate with the agency regarding problems and concerns.
* To comply with State/Federal regulations and agency policies.
* To coordinate scheduling of services with office staff.
* To complete documentation accurately and timely.
* To respect and maintain client confidentiality at all times.
* To report concerns of client abuse or neglect.
* To refrain from discussing personal problems or opinions with clients.
* To establish and maintain a therapeutic relationship with clients.
* To maintain access to email for updates and communication.
* To maintain a clean, insured vehicle for client transportation.
* To work within the goals set in the individual service plan.
* To familiarize and work within the policies of the service.
* To undertake further training as required.
* To work flexibly outside normal hours when needed.
* To maintain confidentiality of sensitive information.
* To manage time effectively to meet deadlines.
* To attend supervision and meetings.
* To undertake other duties as appropriate.
Qualifications:
* A minimum of a BA in a Human Service-related field preferred.
* Ability to research and identify sources of support and guidance.
* Good IT skills, including managing websites and social/media communications.
* Strong listening and communication skills.
* Ability to produce reports and statistics.
* Ability to travel independently, preferably by car.
* Flexibility to work evenings or weekends as needed.
* Impartial and non-judgmental attitude.
* Knowledge of community support systems in the Greater Boston area.
* Committed to the rights of seniors and disabled to live with dignity.
* Reliable, sensitive, and able to handle emergencies.
* Prior home care experience is helpful.
