MACMILLAN SECONDARY BREAST CANCER CLINICAL NURSE SPECIALIST
PART TIME 22.5 HRS/30 HOURS PER WEEK, WORKING 3 DAYS / 4 DAYS ALTERNATE WEEKS
An exciting opportunity has arisen for an experienced cancer nurse to work as part of a job share, leading and developing the Macmillan Secondary Breast Cancer nursing service in line with national directive.
The post holder will be a core member of the breast cancer multi-disciplinary team, coordinating the patients’ pathway, acting as the main point of contact, providing specialist information, advice, and support from the point of diagnosis of secondary disease and throughout the illness experience.
The post holder will be responsible for implementing the key elements of personalised care, ensuring patients are offered a Holistic Needs Assessment/Care Planning at key stages in the pathway in line with national directives, have access to an End of Treatment Summaries after each modality of treatment and health and wellbeing to meet their individual needs.

Detailed job description and main responsibilities
1. To undertake a comprehensive holistic assessment of patient’s needs (HNA) at key stages during the illness experience from the point of referral in line with National directives.
2. To attend significant outpatient consultations, acting as the patient’s advocate, communicating complex information with patients and their families.
3. To review inpatients admitted with complications related to their disease/treatment, influencing management options and liaising with professionals across primary and secondary care to ensure safe effective discharge from hospital.
4. To agree a plan of care in consultation with the patient/family, ensuring a written copy is available for the patient.
5. To attend the weekly cancer multidisciplinary team meeting, contributing to the discussion, providing expert nursing opinion on the patient’s assessment and care and advocating for the patient and family.
6. To screen patients for psychological distress using a recognised tool.
7. To be skilled in providing Level 2 psychological support for patients and their carers, referring patients with complex needs to the Macmillan Clinical Psychologist.
8. To routinely discuss diagnosis and prognosis with patients and their family.
9. To provide specialist support for patients and their families, ensuring their physical, psychological, social, emotional, and spiritual needs are identified and met.
10. To lead on complex communication issues and co-ordinate the patient’s pathway acting as the main point of contact or responsible for nominating a main point of contact for the patient depending on their individual needs.
Person specification
Education, Training, Qualification
* Relevant Master’s Degree or equivalent level of education, training or experience.
* Fundamentals of Cancer Care module post graduate level or equivalent level of education or training.
* Training to provide Level 2 Psychological Support.
* Advanced communication skills training or equivalent level of education or training.
* Registered independent nurse prescriber or willingness to undertake.
* In-depth knowledge and experience in caring for patients living with secondary breast cancer.
* Experience of multi-professional working.
* Knowledge of the National Frameworks related to cancer care.
* Previous experience of working as a Clinical Nurse Specialist.
