Job summary
Due to maternity leave, an exciting opportunity has arisen for an enthusiastic and experienced paediatric nurse to join our dynamic, forward-thinking and dedicated paediatric hospital to home team for a fixed maternity cover. This post offers the prospect to work within an innovative model of nursing care - the PATCH team "Providing Assessment and Treatment for Children at Home. This is a nurse led service providing care in the form of telephone reviews and/or home visits for further assessment to children who would have otherwise been admitted to hospital.
PATCH aims to deliver health education and promotion to children and their families at home during an acute episode of illness to reduce PED re-attendances and help build resourcefulness within the community.
In addition, PATCH continue to nurture existing working links between the Paediatric ED (PED), Paediatric Assessment Unit (PAU)ED, and community teams to ensure seamless pathways for children mild-moderately unwell with common conditions.
The PATCH team will regularly audit and review existing pathways of care for common minor conditions and deliver and disseminate associated educational resources to professionals working across the urgent care pathway for children.
