Clinical Responsibilities Provide high-quality consultations, assessments, and treatment plans for registered patients, managing both acute and chronic health conditions. Undertake telephone, face-to-face, and online consultations across the practices. Issue prescriptions and follow up on test results and referrals as required. Provide continuity of care by managing a personal caseload when appropriate. Participate in the management of long-term conditions, health promotion, and disease prevention. Work collaboratively with the multidisciplinary team, including clinical pharmacists, Mental Health Practitioners, and social prescribers, to support holistic patient care. Contribute to clinical audits, quality improvement initiatives, and population health management projects. Support the delivery of Enhanced Access services and national health initiatives. Maintain accurate medical records following GMC and practice protocols. Participate in team meetings, clinical supervision, and CPD activities to ensure continuous professional development. Work with other healthcare providers, including secondary care and community teams, to coordinate care and ensure seamless patient pathways. Home visits.
