Job Title: Qualified Social Worker – Hospital Team Location: Barnet Rate: £32 per hour An exciting job role has recently become available in Barnet. We are looking for a new member for their Hospital Team. What will your responsibilities be? You will educate patients and their families about illnesses, treatment plans, and available resources. You will conduct psychosocial assessments to identify any mental or emotional distress and address these issues. You will provide counseling to patients and families in crisis or experiencing distress. You will develop and implement individualized care plans to ensure safe and effective discharge from the hospital. You will work with interdisciplinary teams, including doctors, nurses, and case managers, to coordinate patient care and ensure seamless transitions. Additionally, you will advocate for the best interests and wishes of the patients, ensuring their voices are heard in care decisions.
