We are looking for a Practice Nurse to provide high-quality, evidence-based care to patients, particularly those with long-term conditions, including the elderly and housebound. This role encompasses direct clinical care, management of specialised clinics, and active participation in infection prevention and control initiatives. The ideal candidate will demonstrate strong clinical judgement, excellent communication skills, and a commitment to continuous improvement.
Initially looking to recruit for a 6-month fixed term but this may be extended.
Closing date set as 16th March, but may be extended until 30th March.
Main duties of the job
The Practice Nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, cervical screening, and immunisations.
You will manage the nursing care of our patients, ensure up to date and accurate clinical records are maintained and QoF reviews completed.
Your role will be varied and may include:

You will be part of an existing team and work collaboratively with the wider team, including GP Partners, other clinicians, pharmacists, specialist and secondary care organisations.
You may be required to visit patients in their own home and support in our care home visits.

Job responsibilities
The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels.
Primary Responsibilities
1. Assess patient needs to ensure appropriate healthcare provision, incorporating evidence-based practice
2. Develop, implement, and evaluate individual treatment plans for chronic disease patients to promote health and well-being
3. Identify, manage, and support patients at risk of developing long-term conditions to prevent adverse health effects
4. Provide routine nursing care in accordance with clinical evidence, NICE, and NSF guidelines
5. Diagnose and manage acute and chronic conditions as required
6. Deliver wound care (e.g., ulcer management, Doppler assessments) to patients
7. Review patient medications to enhance compliance
8. Provide guidance to patients on the use of prescribed and over-the-counter medications, including side effects and dosages
9. Request and process pathology services and results as required
10. Oversee chronic disease clinics, delivering patient care and referring to secondary or specialist care as necessary
11. Maintain accurate clinical records in accordance with current legislation
12. Ensure effective use of SNOMED CT codes by all nursing team members
13. Maintain and utilize chronic disease registers appropriately
14. Develop, implement, and participate in well-woman and well-man clinics
15. Assist GPs with minor surgery when required
16. Prioritize health issues and intervene appropriately
17. Support the team in managing clinical emergencies
18. Recognize, assess, and refer patients presenting with mental health needs
19. Implement vaccination programs for adults and children

23. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
24. Deliver opportunistic health promotion where appropriate

Qualifications
* Registered nurse with NMC
* Postgraduate diploma or degree (Chronic Disease Management)
* Family Planning qualification
Experience
* Experience of working in a primary care environment
