Our PCN comprises of 2 practices- Clements Surgery (Unity) and Haverhill Family Practice. The practices have a combined geographic area across the Haverhill region and surrounding villages.
There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix.
You will be joining an enthusiastic team of clinicians and administrators. You will be well supported with ongoing professional development (training support, CPD, and peer support). The role will contribute to the improving quality of care of our patients.
The post holder will be employed by the Haverhill Primary Care Network but will report to and work for the Clements (Unity Healthcare) Surgery.
Main duties of the job
The Care Coordinator responsibilities can include but are not limited to the following:
* To work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients.
* To support patients to utilize decision aids, help create single personalized care and support plans, in line with best practice.
* To support the Clinical directors in the delivery of the DES specifications.
* To help people manage their needs through answering queries, making, and managing appointments.
* To provide coordination and navigation with the aid of digital tools for people and their carers across health and care services.

Person Specification
* Educated to GCSE grade or equivalent.
* Experience in Meds Management.
