Macmillan Advanced Clinical Nurse Specialist in Palliative Care

The post holder will take a lead role in the advanced clinical assessment and treatment of a diverse caseload of palliative and end of life care patients and their carers, who have highly complex needs in the hospital and community setting. They will undertake clinical assessments and devise management plans and maintain records as an autonomous practitioner.
The post holder will provide expert clinical expertise supporting, mentoring, and guiding more inexperienced members of the clinical team. They will ensure that evidence-based practice is inherent in all aspects of care and treatment and ensure this knowledge is disseminated throughout the clinical area, Trust and externally as appropriate.
The post holder will be based primarily in Salford Care Organisation and work in both the hospital and community setting as part of the enhanced 7-day service.
The Specialist Palliative care team in the Acute and Community are dynamic and forward-thinking. We are caring and supportive teams and have a breadth of knowledge and skills within palliative and end of life care.
Home to the Manchester Centre for Clinical Neurosciences, Greater Manchester Trauma Hospital and the Geoffrey Jefferson Brain Research Centre, Salford is improving care for some of the sickest patients in the region and delivers a range of acute, community, primary care services and specialist services to Greater Manchester and beyond.
Main duties of the job
* Undertake a significant clinical caseload to an advanced professional standard using specialist knowledge from theoretical and practical experience.
* Triage referrals either independently or with medical colleagues.
* Work across organisational boundaries providing face-to-face on-call cover to enhanced palliative care 7-day working.
* Act as a senior clinical decision maker providing advice to the Clinical Nurse Specialists and ward teams in relation to complex situations.
* Develop and maintain communication with people about difficult and complex situations.
* Apply knowledge of advanced communication skills in order to effectively break bad news or convey uncertainty in a range of formats, written or verbal.
* Develop the skills required to practice at an advanced level and to use an extended scope of practice beyond own profession (including prescribing and examination skills, advising on ethical issues).
* Plan, deliver and evaluate interventions and/or treatments.
* Provide Macmillan Cancer Support with an annual report to demonstrate advanced practice and support for patients, carers and staff and participate in service reviews to help drive improvement.

Person Specification
Knowledge, Training & Experience
* Extensive post registration experience with evidence of clinical experience at band 7 or above, managing a caseload of patients with complex palliative and EOL care needs.
* Evidence of extensive expert professional/clinical knowledge within specialty supplemented by specialist clinical training and CPD.
* Effective communicator, able to communicate complex and highly sensitive information.
* Evidence of relevant involvement in meeting the Trust clinical governance objectives.
* Evidence of involvement in the development of programmes of care, protocols and clinical audit.
* Evidence of involvement in leading service development.
* Excellent clinical reasoning/clinical decision making.
* Evidence of involvement and leadership in teaching and mentoring learners.
Qualifications
* Current NMC registration as a first-level nurse
* Degree in Nursing
* Relevant Post-basic qualification
* V300 Non-Medical Prescriber
* Enhanced Clinical Skills module or equivalent
