SUMMARY Responsible for overseeing the Medicare program and assure compliance with all aspects of it, including maximization of benefits for the resident, timeliness of the MDS completion and transmission of the MDS. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Facilitate, monitor, and evaluate resident plan of care. Participate in admission decisions. Coordinate gathering and sharing of clinical information for Medicare MDS and RAI process. Manage MDS assessment schedule by establishing and coordinating the best assessment reference dates. Coordinate with rehabilitation services. Ensure timely assessment and data submission. Monitor MDS data entry. Monitor patient RUG categories. Provide RUG and HIPPS information to business office. Monitor documentation to support MDS and Medicare claims (nursing and therapy). Monitor documentation to ensure acceptability for HCFA medical review process. Ensure physician orders meet Medicare guidelines for skilled services. Ensure PPS policies, procedures and regulations are being met using web site resources to keep current and access information. Communicate on a frequent and timely basis with facility management on Medicare and MDS related topics. Make daily rounds on Medicare patients. Communicate with nursing and rehabilitation staff daily regarding status of Medicare patients. Responsible for pre-admission through discharge of resident for accurate flow of information to all parties involved. Review admission orders for order to admit to Medicare skilled services. Initiate the Physician Certification/Recertification forms and insure its timely completion at the appropriate intervals. Conduct weekly Medicare meetings and maintain minutes of meeting. Place charting guidelines for each Medicare resident in front of the nurses documentation section of medical record. Insure charting guidelines are kept current. Tag the chart that the resident is in a transitional status and for 30 days following discharge from Medicare the resident may be reinstated if there are days remaining in the benefit period and the resident's condition changes and the daily skilled services are again required. Make daily rounds to observe and assess each Medicare resident. These rounds are to be completed before the stand-up meeting. Review each resident's condition daily to assess appropriateness of Medicare coverage. Review midnight census for accuracy. Coordinate with the Admissions Coordinator's census sheet. Report any discrepancies to the business office. Other Medicare duties as necessary and appropriate. Participate in care planning process. Maintain up-to-date care plans on all residents residing at the facility. Conduct quarterly and annual assessments using designated format. Advise and effectively communicate the plan of care to nursing staff and ensure that the plan is being followed. Qualifications/Experience Associates or Bachelor's degree in Nursing and licensed in the state of NC. Several years of experience as a MDS Coordinator is required. Experience working in LTC, SNF is required. Will also consider an LPN for the MDS Assistant Coordinator with MDS experience. Must be licensed in the state of NC as a LP N. Work Schedule One Full Time position available, Day Shift Hours 40 hours per week, Monday - Friday. Some additional hours in the evenings and on weekends may be required to meet the business demands of the position.
