We are looking for an enthusiastic and motivated Band 6 Community Psychiatric Nurse to join our dynamic Community Mental Health Team. In this role, you'll provide high quality, compassionate care to adults with functional mental health conditions. You'll work collaboratively with a well established team to deliver personalised treatment plans, conduct comprehensive assessments and provide support to patients and carers as they navigate complex mental health challenges.

Main duties of the job
To work with adults experiencing functional mental health concerns, and their families, in the Adult's Community Mental Health Team within East Cheshire. The post holder will hold a professional qualification and have relevant experience regarding adult mental health care.
The post holder will be responsible for:
1. Undertaking assessments and coordinating the care of service users.
2. Working closely with family members to maintain high standards of collaborative care.
3. Actively contributing to the day-to-day operational functions of the team such as completing new assessments, covering office "duty" and working as a team to deliver the team's goals.
4. Developing and maintaining positive working relationships with service users, their carers, General Practitioners, third sector partners, other significant professionals, and members of the multi-disciplinary team.
5. Ensuring that clear channels of communication are established and maintained with clients, carers, GPs, Social Services and other partners.
6. Promoting and identifying areas requiring development within the CMHT in the Cheshire East area and taking a lead role in the development of services in conjunction with the Team Operations Manager, Head of Clinical Services and other professionals.

Person Specification
Qualifications
* Recognised qualification in mental health
* Evidence of further professional development involving serious mental illness.
* Experience of managing other staff.
* Training in p.s.i.
Knowledge & Expertise
* Knowledge and sensitivity to equal opportunities issues
* Sound knowledge of current professional staff development and continuous service improvement issues, particularly in the context of Adult Mental Health Services
* Knowledge of developments in Community mental health from a political and clinical perspective.
* Sound knowledge of the Mental health act and other legislation i.e. Dols
* Knowledge of the physical aspects of serious mental Disorder.
* Understanding of assertive outreach approach to care.
* Information management and technology skills
* Experience of working within an assertive outreach framework.
Experience
* Significant post registration experience, in addition to some experience working in Community Mental Health Services.
* Experience of multidisciplinary working
