Job summary To be responsible for the care delivery for adults with palliative and end of life care needs. Lead the palliative support team on a daily basis. Work with the team to promote the delivery of seamless care to all patients registered with the service. To foster effective working relationships and processes between other NHS Trusts, General Practitioners, Primary Health Care Teams, Local Authority, Independent providers and Voluntary Services. Collate activity and performance data and formulate service reports as required. Be a proactive and visible leader for the Palliative Support team. Be responsive to the changing needs of patients, their families and carers ensuring appropriate assessment and review of the health needs of those who require supportive end of life care. Take an active role and work as an assessing clinician in facilitating and or providing professional support in the assessment process. Support the work of the Head of Service, Co-ordinating lead, rapid response and the Specialist Palliative Care Team in delivering high standards of end of life care. Main duties of the job To be responsible for the clinical development of the palliative care support team in the effective delivery of care packages & highly skilled nursing care for patients with complex palliative care needs. To have day to day management responsibility for the Palliative Care Support Team To work in collaboration with all appropriate services to facilitate a rapid, seamless discharge to enable PPOD.
