Job summary
On a daily basis the Advanced Clinical Practitioner (ACP) will provide clinical assessment and management to patients.
The post holder will be autonomous in decision making, including judgements involving complex facts/situations that require analysis, interpretation and comparison of a range of options in relation to the care & management needs of the patients on a day to day basis within the clinical environment.
The post holder will support research/use information systems, lead and participate in relevant research within practice, as well as demonstrate an understanding of research methodology.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
This post is fixed term/secondment for 8 months to meet the needs of the service.

Main duties of the job
1. Undertake comprehensive clinical assessments for patients with falls andosteoporosis, which may include physical examinations and comprehensive patient histories and accessing previous radiological investigations, GP letters and clinic letters.
2. Post holder will be clinically supervised and supported by the Clinical Lead FLS Consultant
3. Undertake virtual and face to face clinics with support of FLS Clinical Lead
4. Working closely with various specialties including COTE, Rheumatology, Orthopaedics & Primary Care
5. Work collaboratively with Multidisciplinary team and able to lead the team, if required.
6. Post holder will be trained as needed to deliver specialist bone health treatment in the community and expected to deliver 'care closer to home' and link with GP surgeries.
7. Utilise critical thinking and problem-solving skills including critical reflection.
8. Manage complexity and develop higher levels of autonomy
9. Assess and manage risk within the care environment
10. Refer patients for relevant investigations and tests required to assist other clinicians in diagnosis
11. Develop and implement therapeutic interventions including non-medical prescribing and referrals to others as appropriate
12. Play a key clinical role in supporting & developing the Fracture liaison & Bone Health service, this will include leading in aspects of audit and evaluation in collaboration with the Clinical Lead
13. The post holder will use their expertise for the supervision, and training of other staff members.

Autonomously examine, treat and support patients who have sustained falls or living with impact of fall, particularly fragility fracture. Promote evidence-based practice and act as an accountable role model. Ensure patient pathway is effective and timely. Responsible for the provision of effective, efficient and appropriate high quality care for patients who have experienced falls in the community. To manage fragility fracture (explaining diagnoses, sharing treatment options and monitoring compliance to treatment and its side effects) and participate in the mandatory Fracture Liaison Service - Data Base (FLS-DB). To complete follow-up as per FLS-DB key performance indicators at 16 weeks and 52 weeks. The post holder will see patients within hospital and community settings and act at all times in accordance with the postholders Professional Code of Conduct and CTM policies. Provide responsive, flexible and patient centered services in line with NICE guidelines for the management of Falls and Bone Health. The post holder will be supported by the Clinical Lead FLS Consultant.
