Anexciting full time/part time opportunity has arisen to recruit RegisteredNurses to work at Darlington Memorial Hospital or University Hospital of NorthDurham within the Discharge Lounge. We strive to provide an effective andefficient service to our patients who require our input in assisting with theirdischarge Discharge Lounge provides a warm caring environment in whichpatients can safely wait for discharge home or transfer to an alternativesetting such as community hospital within the Trust.
Excellentcommunication skills are essential to ensure a safe discharge and an increasein patient flow around the hospital. The postholder will be required tocommunicate with the Wards, Doctors, Pharmacy, Relatives, Carers, and Transportand Patient Flow Team. The postholder will supervise discharge lounge HealthCare Assistants, liaise with the Discharge management team/ward coordinators andrelatives. The postholder will be responsible for patient care during theirtime in the discharge lounge and efficient coordination of booking patienttransport and checking prescribed medication in readiness for safe discharge.
Main duties of the job
Thepostholder will supervise discharge lounge Health Care Assistants, liaise withthe Discharge management team/ward coordinators and relatives.
The postholder will beresponsible for patient care during their time in the discharge lounge andefficient coordination of booking patient transport and checking prescribedmedication in readiness for safe discharge.
The postholder will requireeffective communication skills to create and build a good network ofrelationships with all wards and departments.
Thepost holder will have a nominated base and be expected to travel and workacross both sites, if required, to meet service need during times of pressure.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
