Responsibility for Patient/Service User Care To be accountable for the delivery of clinical services ensuring a high standard of carebased on best practice to maintain patient/service user safety and provide anexcellent experience. To coordinate the assessment of patients/service users with complex needs includingindividuals with unconfirmed diagnosis. To coordinate the development, implementation and evaluation of specialisedprogrammes of care utilising evidence and research. To manage a service area/provide highly specialist advice in acute and chronic situations ensuring that the patients/services users needs are met along with theirsupport network. To ensure that all staff involve patients and service users in the planning, delivery andevaluation of care, respecting their beliefs and dignity. To utilise data and information from a range of sources to identify patient/service usercondition to guide decision making. Within field of expertise and policy and where appropriate, request specific clinicaltests to establish a diagnosis. To undertake the role of health promoter and educator for patients/service users andtheir support networks. To ensure that professional standards are upheld by the staff members working withinthe postholders sphere of responsibility. To manage all incidents as per policy and participate in risk management and Qualityand Safety processes e.g SWARM. To be accountable for ensuring that safeguards are in place for patients/service usersand escalate any safety, safeguarding or risk concerns following the appropriateprocesses. Communication To build and advocate a therapeutic relationship with patients/service users andtheir support network, communicating with them in a way that respects their views,language, cultural differences and cognitive function. To use and promote a wide range of communication skills that can be adapted to meet the needs of complex and challenging situations. To manage conflict between individuals including staff, patients/service users and their support network. To establish and promote effective working relations with all members of the multidisciplinary team and outside agencies, clearly relaying condition and service-related information in a timely and effective manner. To communicate with patients/service users and support networks compassionately, in a confidential and sensitive professional manner. To produce clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards. Leadership To act as a professional role model for staff, advocating for patients and service users and promoting high standards of practice and strong professional values. To manage the service area and coordinate HSC and multi-agency activities. To ensure the timely investigation of complaints and clinical incidents related to the service area, ensuring that learning is shared with own team and wider HSC. To promote own and team success and excellent standards of care by reflecting, challenging others constructively, upholding HSCs CARE Values. To ensure team members are aware of organisational objectives, policies and procedures and implications that they have for their practice. To undertake Values Based Appraisals for staff and regular one-to-ones and support such staff with performance issues, developing action plans to address developmental needs and organisational objectives. To monitor and review staffing levels, skill mix and recruitment and retention trends in line with future service developments and initiatives, providing input into Strategic Workforce Plans Planning and Organisation To shape and implement a vision for future service development and improvement, and to critically analyse complex information and evaluate a range of options. To encourage and facilitate new ways of working that support more efficient and focused patient/service user centred care. Policy and Service Development Responsible for policy and service development ensuring that new initiatives are incorporated into the working practices of staff. Partnership Working To demonstrate an understanding of and apply the principles of the CARE Values. To ensure the delivery of a collaborative working approach as part of a multidisciplinary team with a range of professionals from other service areas. To lead effective team working, communicating with others in a constructive way, which enables colleagues to make suggestions for the improvement of care delivery. To lead and participate in team meetings. Responsibility for Physical and Financial Resources To ensure that public funds are used responsibly and appropriately. To authorise expenditure, e.g. the signing of timesheets, authorising the use of additional staff and purchasing of supplies. To ensure the safe and responsible use of equipment and participate in the procurement process for the replacement of high value equipment. To manage service area budget, if appropriate, and report any actual or potential deviations from the budget to the senior manager. To ensure all staff handle valuables in accordance with agreed policies and procedures. Professional Development To take responsibility for maintaining own professional development and revalidation requirements, and that of others within the service, ensuring completion of training in accordance with the Statutory, Mandatory and Essential to Role Training Policy. To participate in and lead supervisory one-to-one meetings and HSCs Values Based Appraisal process, developing plans to meet personal and organisational objectives. To continually develop new knowledge and skills, applying this to practice and sharing with colleagues as appropriate. To ensure that programmes of learning are developed to meet the educational needs of staff working within the department or service. To participate in and develop plans for the induction, orientation and development of new staff, agency workers, students and any voluntary workers where appropriate. To ensure the service area provides an effective learning environment and to supervise and assess pre and post registration students in accordance with University and HSC requirement
