Job Summary
Intermediate care is a service delivered in collaboration with our local authority partner. Working in partnership with the bed-based manager, the post holder will:
1. Be responsible for the clinical management of the Intermediate Care Nursing Team, ensuring effective and efficient delivery of a high-quality timely service to meet commissioner requirements.
2. Undertake highly complex clinical assessments and ensure the nursing team provides care in line with Trust and professional body standards.
3. Plan capacity and demand to deliver a safe service and contribute to admission avoidance and a timely discharge home.
4. Motivate and support the nursing team, inclusive of supervision and staff development to ensure a high standard of care and support to patients.
5. Carry out the duties ensuring the service promotes equality and diversity, offering person-centered care with dignity and respect.
6. Be responsible for the implementation of partner agency and Trust policies and procedures.
Main Duties of the Job
1. Work collaboratively with Intermediate Care Service and Stakeholders to ensure patients' needs are met.
2. Provide leadership and support by promoting professional development and encouraging clinical supervision.
3. Ensure that capacity is proactively managed to meet the demands of the services.
4. Promote good team working and good working relations amongst staff.
5. Ensure timely accurate information is provided on annual leave, expenses, and study leave.
6. Monitor and report to line manager all HR, safeguarding, and health and safety issues discussing and taking action as appropriate.
7. Actively manage sickness and absence of staff accessing support from their line manager as applicable.
8. Ensure effective use of physical and financial resources including the ordering of supplies and equipment.
9. Be responsible for the equitable distribution of rotas and prioritization of workload.
10. Monitor, report, audit and evaluate that staff maintain appropriate and accurate patient records.
11. Oversee the collection of all monitoring data and contribute to performance management.
12. Lead on the clinical aspect of complaints, taking action where appropriate to ensure learning as a result of patient/service user feedback.
13. Investigate incidents relating to nursing practice, ensuring that learning takes place across the service.
14. Assist their line manager to recruit, motivate, develop, support and induct staff with policy and service priorities.
15. Responsible for own time management and that of the nursing team.
16. Flexibility to participate in rota cover to meet the service needs.
17. Manage time to ensure a split of 80% leadership/management and 20% clinical duties to ensure maintenance of clinical skills.
Clinical Responsibilities
1. Complete comprehensive assessment of highly complex patient nursing needs. Plan, assess, implement and evaluate care delivery according to changing health needs using prescribing skills.
2. Ensure the nursing team's practice is evidence-based through undertaking audit and critically appraising literature to achieve a high standard of care.
3. Actively contribute to multidisciplinary meetings.
4. Establish and maintain effective communication with individual groups and communities in complex and potentially challenging situations.
5. Involve patients and carers in the planning and delivery of care.
6. Competently undertake as required a range of clinical skills related to the post.
7. Maintain accurate records in accordance with NMC and Care Quality Commission standards.
8. Identify skills and knowledge to address self and individuals training needs by undertaking PDR.
9. Demonstrate the ability to work without direct supervision and to solve problems in a collaborative and inclusive manner.

1. Lead audit activity as set out in the service quality assurance framework to ensure high-quality care is maintained and where required, learning is embedded.
2. Offer advice and support to others as appropriate, acting as a positive role model for all staff.
3. Address shortfalls in performance, escalating concerns to the service manager where required and in line with Trust policies.
4. Lead and participate in protected learning time and attend staff meetings as requested.
5. Maintain competency to practice through Continuing Professional Development (CPD).
6. Contribute to own CPD in a positive manner, evaluating one's own practice and participating in the appraisal process. Attend relevant training courses and mandatory training.
7. Cascade policy documents within the team ensuring their implementation and understanding.
8. Positively represent and promote the Intermediate Care Service.
9. Lead and engage as required in specific projects, meetings and work arising from the responsibilities of the team manager in the wider delivery of service provision.
10. Lead the team's professional development and participate in individual Performance Development System.
Person Specification
Qualifications
Essential
* RN First Level.
* Professional knowledge acquired through degree supplemented by diploma level specialist training, experience, short courses.

* Degree or master's level qualification.
