Step Together is a trailblazing service that offers a psychological, adversity and trauma (PAT) informed approach to those rough sleeping/at risk of homelessness with multiple, complex needs (mental health, forensic history, substance use) in Somerset. We advocate for system change, creatively thinking with other agencies to develop innovative ways to overcome barriers to engagement, turning these into opportunities to steer positive trauma-informed system change.
Main duties of the job
Working to the recovery principles and Step Together's psychological, adversity and trauma (PAT) informed model, you will engage and support your own caseload of clients in identifying and overcoming barriers to maintaining their accommodation. The focus of these fixed term roles is to support Somerset's Housing & Homelessness services to prevent homelessness, and to reduce the need for individuals to receive intervention from services in the future. You will have an understanding of issues relating to homelessness and mental health, and be able to build and sustain relationships with clients. You will have a commitment to promoting independence and supporting vulnerable people in developing move on and recovery plans. You will have knowledge and experience in the prevention of homelessness, including working with local authority and housing partners to support clients at risk of homelessness to maintain their accommodation. We work in a recovery focused, psychological, adversity and trauma (PAT) informed way in all aspects of the team's practice using relevant interventions, including Assessments, Support Planning (using the Outcomes Star), Risk Assessing, Brief Interventions and Solution Focused Therapy.

Job responsibilities
1. Conduct and support client assessments.
2. Positively manage and reduce clients' risk using safety and inclusion planning tools.
3. Work within Step Together's psychological, adversity and trauma-informed model to provide relational support to individuals who may have complex needs.
4. Working with a caseload, undertake proactive work across Somerset to engage and build trusting relationships with clients, developing a full understanding of their backgrounds, current situation, priorities, needs, hopes and ambitions.
Person Specification
Experience
* Proven experience of working in service provision for people with complex and multiple needs relating to at least two of the following: mental health, drug and alcohol, homelessness, offending.
* Proven experience of outreach or floating support work, demonstrating persistence and flexibility and a range of strategies in successfully supporting clients to engage voluntarily.
* Proven ability to build relationships, and to work collaboratively and successfully in a team environment across sectors and agencies.
* Proven ability to promote and apply good practice in equality and diversity.
* Proven ability to operate risk management and safe working practices in working with vulnerable clients.
