Job summary The Emergency Department is looking for a highly-motivated, reliable and enthusiastic individual to join the team as a Patient Tracker. Your role will be to ensure accurate, timely data input and flow support is provided to the wider team to prevent unnecessary delays and improve patient experience. You will also escalate potential delays in a timely manner, working closely with doctors, nurses and support staff. Main duties of the job To work primarily independently and on own initiative with minimal supervision to carry out the duties of the post. Take responsibility for the prioritisation of workload. Respond to changing priorities and conflicting demands in an effective manner and develop systems to improve effectiveness of the post. The role involves long-day working (i.e. 07:00-19:15 or 19:00-07:15). This will include days/nights, weekends and bank holidays. To actively monitor and manage the pathway of patients in the Emergency Department to ensure that the patient journey is managed within the relevant time-scales. Review and track patient information and length of time in the department on the ED IT system (Symphony) ensuring that any delay in the pathway is escalated in line with agreed protocols.
