Job summary As Wellbeing Support Coordinator, you will ensure the smooth running of the service which is available to anyone receiving support from Weldmar Hospicecare. You will work in collaboration with our Patient and Family Support Team and those from the wider multi-disciplinary team and will be responsible for the day to day coordination and delivery of a range of activities of the Wellbeing Support Sessions, with line management responsibility for the Wellbeing Support Workers, and Volunteers. Working with the Patient and Family Support Lead, you will identify service development opportunities, and lead on the implementation of these. Main duties of the job You will be responsible for processing referrals to the Wellbeing Support Sessions, considering operational capacity and logistics, while balancing needs and ongoing eligibility of those accessing the activities programme, as well as access to additional services such as counselling, spiritual support and complementary therapies to ensure the highest standard of Wellbeing Support. You will ensure that patients, carers and those experiencing bereavement can come together in a relaxed and informal environment, where they will have the opportunity to build friendships and a network outside of the sessions supported by those experienced in providing emotional support. Experienced in working with patients needing End of Life Care, you will have an NVQ level 3 or equivalent in a care related field and will use your excellent communication and interpersonal skills to manage the physical, emotional, psychological and spiritual needs of patients, their carers and families in a group setting.
