Main duties of the job Our Integrated Community Services Teams are enthusiastic with a breadth of experience and a shared goal to offer the highest standard of care to our patients. All team members are expected to carry their own clinical caseload, attend regular therapy meetings and undertake individual and joint therapy assessments and treatment sessions with members of the MDT where appropriate. Rehabilitation is delivered to patients in their own homes with a focus on improving health and well-being, working toward patient-centred goals and preventing hospital admission or facilitating discharge home.
