The SWAT team work across athe acute and community Orthopaedic pathway and is focused on multidisciplinary team working including therapy advice and wound management. The SWAT Team, comprised of Nurses, Physiotherapists, Clinical Support Workers and Technicians, facilitates joint replacement patients throughout their journey with support, guidance and treatment. The Orthopaedic department has been recognized nationally as one of the top performing centres, undertaking around a 1,500 hip and knee joint replacements a year. We have successfully implemented a day case pathway to include total hip replacements and knee replacements. Main duties of the job As part of your role you will mostly support the SWAT Team managing patients in the community however will also work with the elective ward and, after training, have the option to support wider services. Training will be provided to help you to develop clinically and with leadership skills as appropriate. The role will involve: Delivering a high standard of care to orthopaedic patients undergoing trauma or elective surgical treatment. The post holders will be responsible for the assessment of patient care needs, and implementation and evaluation of the programme of the care both in the acute setting and in the community as part of the SWATT enhanced recovery program.
