The ethos is to focus on supporting patient’s home earlier from hospital where a Nursing need has been identified or a Fastrack application The successful candidate will lead and manage the team to ensure a seamless service is provided to ensure safe and effective discharge planning of complex patients within the Central IHC You will have at least 5 years experience as a complex discharge nurse and be able to coordinate and supervise the team to ensure a patient centred service. This is an exciting and challenging role for an experience individual who is dynamic, possessing strong problem-solving skills, with the ability to deal with difficult and sensitive situations within a busy and challenging clinical setting. Be able to deliver a Discharge training programme to new starters. Main duties of the job The post holder must have an extensive knowledge and understanding of The Discharge Process, Funded Nursing Care and Continuing Healthcare. as well as the Fastrack process You should also have excellent communication, assessment and documentation skills and be committed to working within a multidisciplinary environment. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
