Job Overview
The High Peak North Adult Care Team are based in Glossop and we are seeking to recruit an experienced Social Worker to join our friendly, community-based team of Social Care Practitioners and Social Workers. The post is for 4 days a week, between Monday and Friday.
The team is highly supportive and operates on a largely agile, flexible basis across the High Peak area of Derbyshire, utilising link worker and champion roles to carry out Care Act (2014) face-to-face and telephone assessments and reviews.
Key Responsibilities
1. Evidence high levels of autonomy and accountability with timely recording of interventions to progress overall case management.
2. Build positive, professional links with partner agencies such as Health, Police, Housing, and external sectors.
3. Identify signposting opportunities to support carers and maximise independence and individual wellbeing.
4. Provide Duty cover, responding to changing situations, and effectively triaging and assessing incoming work.
5. Manage a complex caseload while meeting statutory duties in a safe, accountable, and cost-effective manner.
6. Lead on Section 42 Safeguarding, chair multi-agency meetings, and complete Mental Capacity Act (2005) assessments.
Qualifications and Experience
It is an advantage to have experience in the areas mentioned above, though support, training, and experience will be available for the right candidate if you wish to develop your skills in these areas.
