CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
We are looking for two Advanced Clinical Practitioners who are passionate about delivering high quality and effective care to join our Unplanned Care services in South and West Hertfordshire.
You will be working with a diverse MDT including a Nurse Consultant, other ACPs, nurses, paramedics, therapists, GPs, support workers and administrators who work across a number of innovative unplanned care services, all aiming to keep patients out of hospital and be discharged home more quickly. These roles will be integral to increasing our prevention of admission and early supported discharge numbers, and building relationships and working closely with internal teams, primary care colleagues and nursing and residential homes to ensure referrals into our services are maximised.

Our virtual ward and Hospital@Home services are delivered in collaboration with our acute Trust colleagues at West Hertfordshire Teaching Hospital NHS Trust.
Main duties of the job
One of these roles will be delivering advanced clinical decision making in our Rapid Response and Early Intervention Vehicle services, the other will be helping to expand the prevention of admission and step down from hospital offers for the new General Medical Virtual Ward, helping our residents to stay and be treated safely at home, and be discharged from hospital more quickly.
The roles will adhere to the four pillars of advanced clinical practice and will be complementing the two existing ACPs in our Frailty Hospital at Home service, working on a rota system to ensure robust weekend and bank holiday cover for advanced clinical decision making.
You will need to demonstrate effective, proven clinical leadership and advanced clinical decision making skills alongside an ability to build positive, productive relationships with teams and external stakeholders. These are full time positions that require working shifts including weekends and bank holidays. The service operates under pre-rostered shift patterns spanning 8am - 10pm, 7 days per week.

Job Purpose
The post holder will be practising autonomously as an Advanced Clinical Practitioner (ACP) to provide patient centred quality care. This will encompass the skills of assessment, examination, diagnosis, and treatment within an agreed scope of practice throughout the Unplanned Care services. The post holder will support a way of working that emphasises a more efficient and patient focused service and will ensure the safe prevention of admission and discharge from hospital of patients with undifferentiated and undiagnosed presentations across Unplanned Care pathways. The post holder will deliver both a clinical and leadership role with a focus on development of the service and research, along with management responsibilities.

Education/Qualification
Essential
* Registered Nurse with current NMC registration or Physiotherapist or Paramedic with HCPC registration
* MSc / PGdip Advanced Clinical Practice
* Independent Non-Medical Prescriber
* Evidence of Management/ Leadership training
* Evidence of teaching demonstrated by a suitable qualification or accredited experience
Desirable
* Evidence of continuing professional education and training at master's level.
* Further desirable qualifications in Primary Care/ Accident & Emergency
* Recognised qualification in Minor Injuries
Experience
Essential
* Extensive experience in Community Care and/or Primary Care and/ or Accident & Emergency
* Experience in a clinical leadership/ management role within a complex healthcare environment
* Extensive theoretical & practical knowledge relating to assessment & treatment of minor injuries and illnesses
* Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled medical care and management of patients presenting with undifferentiated and undiagnosed conditions
* Experience of assessing and providing quality care to people with Long Term Conditions
* Track record of practice excellence
* Proven ability to prioritise and manage own workload with minimal supervision.
