Full and part time positions available up to 60 hours.
Working within Eden's GP practices and closely with the wider ICC team, the Living Well Coach (LWC) will work within the Wellbeing Service to assess individuals with LTC, Frailty, and psycho-social needs, engaging them to set personalized goals and access services and resources best suited to meet their needs.
The assessment might include basic health assessment, low-level clinical screening, wellbeing assessment, review of functional abilities, assessment of social/housing/employment circumstances, and understanding of social and support networks.
LWCs will develop skills in active listening, motivational interviewing, and appropriate psychological care skills including CBT, acceptance & commitment therapy, and mindfulness. Training and ongoing clinical supervision will be provided by the Physical Health Psychology Department within North Cumbria Integrated Care NHS Foundation Trust (NCIC).

Main duties of the job
Using their coaching skills to support individuals identified by the GP practice and wider Integrated Care Community (ICC), Living Well Coaches (LWC) work alongside people who want to improve their self-management skills in health and wellbeing. The coach will listen to what is important to the person and help them identify and commit to step-by-step activities towards their improvement goal.
Although the base will be within one/two GP practices in the Eden Integrated Care Community (ICC), the post-holder will be expected to work in various environments, including visiting people in their own homes, in other GP practices, or community health/care settings. Individual LWCs will be allocated to one or two practice(s) allowing continuity for patients and direct engagement with the GP practice team.

Experience
* Experience of working within a team
* Experience of organizing own day-to-day work and adjusting priorities in response to changing needs

Qualifications
* A qualification in Health/Social Care or an accredited award such as NVQ Level 3 or prior work experience of similar work
* Care Certificate or the willingness and ability to undertake (applicable for all non-registered support workers that provide health care)
* ECDL or equivalent
Knowledge
* Understanding of the needs of vulnerable people.
* Able to recognize where individuals might be at risk and take appropriate action
* Knowledge of Health & Safety procedures and practices when working with clients and carers in various settings.
* Core competencies in the use of MS Office and data entry, including creating, updating, and amending care plans and assessments.
* Commitment to policies in relation to Equal Opportunity, Customer Care, service delivery, and data sharing, and the ability to implement these policies in the workplace.
* Experience of using EMIS and PRIMIS information systems
* Working knowledge of the range of psychological and physical health problems affecting adults.
