What does the role involve:
14. Carrying out full risk assessments and a person-centred outcome-based care plan for each new client
15. Complete quality monitoring visits with your clients
16. Completing regular care plan reviews
17. Completing regular MAR chart checks, both on e-MAR and paper MAR charts.
18. Rostered “On Call” duties, ensuring that our service runs smoothly during out of office hours and that all communications are documented and shared internally
19. Liaising with other Healthcare professionals such as Social Workers, Occupational Therapists, GP Surgeries etc
20. Coaching and mentoring new care staff
Our Requirements:
21. You must be a caring and compassionate person who is looking to deliver outstanding care and work within the team
22. You must the ability to deal with problems and final solutions, and be able to work with a pressured environment ensuring that the team is competent at all times.
23. You must hold a valid UK driving license and have you own transport
24. NVQ level 3 would be desirable but not necessary but you would have to complete whilst on the job
