The Hospital Social Work Team in the East Riding of Yorkshire Council is undergoing some exciting change. We are remodelling the way we work to support the principles of Home First - supporting a 'discharge to recover and then assess' approach that enables people the time and space to recover in the place they call home before we consider long term care options. You will work as part of a multidisciplinary team that includes occupational therapists, community support workers, and assessment officers whose ethos is to maximise people’s independence.
The team covers a very large geographical area and supports a number of hospitals, including Hull Royal Infirmary, Castle Hill Hospital, York District Hospital, and Scarborough General Hospital, as well as out-of-area hospitals including Scunthorpe and Doncaster.
The Team has the benefit of an excellent skill mix of registered and non-registered practitioners, meaning that we can allocate work according to a range of skills and knowledge. We won Team of the Year in our East Riding of Yorkshire Council Adult Social Care Awards in 2022.
The successful candidate will require some experience of undertaking and completing assessments as well as working collaboratively with key partners across the health and care system.
You will be required to support discharges from all our hospitals, and although we practice a blended and flexible approach, you will need to be agile and have a UK driving licence and access to a vehicle. As a non-registered practitioner, you should be able to demonstrate knowledge and understanding of relevant legislation and guidance such as the Care Act 2014, the Mental Capacity Act 2005, and the Human Rights Act 1998. You must also have a sound knowledge of a strength-based approach and a drive to support people back to being as independent as possible.
You will work in a supportive, strong, and caring team. You will receive regular supervision and have access to training to develop your interests, expand your knowledge, and become an expert at what you do.
