An exciting opportunity has arisen for an enthusiastic and highly motivated Assistant Practitioner to be a part of the Autism Diagnostic Assessment Pathway Team. You will join an established team of forward-thinking multi-disciplinary practitioners offering person-centred diagnostic assessment services.
The service has been designed to identify the strengths as well as the needs of each young person; supporting them to understand the diagnostic outcome and what it means for them. You will also be working with the local autism community hub. CWP are dedicated to working in partnership to ensure that the unique profile of each individual is understood, and support is tailored. You will be working as part of a team who are committed to improving the long-term outcomes for autistic young people and their families.
The Assistant Practitioner role will help the team to deliver robust and timely autism assessment services.
1. To offer clinical and administrative support to the autism assessment pathway team, working under the supervision of case managers and pathway leads to deliver robust, timely, and person-centred autism assessment services.
2. To support in meeting the needs of the young people and families accessing the service.
3. To support young people aged 6-18 years and their families, to attend their health appointment.
4. To follow up the appointment with relevant professionals.
5. To support efficient pathway flow and effective communication.
6. To access and report relevant information.
7. To support with the co-creation of written and visual reports for diagnostic reviews and meaningful communication of outcomes.
8. To attend relevant meetings.
9. To work with the multi-disciplinary team of professionals to bring about the best possible assessment outcome for young people and families.
10. To support with collation and analysis of routinely collected data and outcome measures related to the experiences of people accessing our services.
You will be supported to agree a working pattern that suits you as well as the service. A flexible mix of in-person and remote working can be established. This allows the best possible service for families and the best possible work-life balance for members of the team.
