Job Description:
We are seeking a dedicated and compassionate full-time Care Coordinator to join the team at BeHappy@Home. This office-based role (Monday to Friday, 9 am to 5 pm) is crucial in ensuring that our customers receive the highest level of care and support. The ideal candidate will coordinate care services, manage customer information, and handle office administration to support effective communication and care delivery across our service.
The ideal candidate will play a vital role in streamlining rota management, customer and staff communication, and service coordination to ensure our customers receive the highest level of care and support. This position requires proven care coordination experience, proficiency in Care Planner and Birdie (desirable), excellent organisational skills, and excellent communication skills both written and verbal.
Key Responsibilities:
1. Collaborate with the team to coordinate and manage rotas, ensuring staff availability aligns with customer needs.
2. Act as a primary point of contact for customers, staff, and healthcare professionals, facilitating clear communication and addressing any changes or concerns promptly.
3. Assist in the planning and scheduling of care services, adjusting as needed to accommodate customer requirements and staffing levels, i.e., accommodating annual leave and sickness.
4. Support continuous improvements in communication and service delivery within BeHappy@Home.
5. Perform office administration tasks.
Requirements:
1. Proven experience in care coordination or a related role within the health or social care sector.
2. Excellent organisational skills and attention to detail.
3. Strong interpersonal and communication skills to liaise effectively with customers, staff, and external professionals.
4. Flexibility to work alternate weekends as part of the shared role structure.
5. Commitment to delivering compassionate, customer-centred care and ensuring a positive experience for both customers and team members.
