Working within your clinical competencies alongside the general practice teams you will prescribe, be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy. Key Areas of Responsibility To proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with co-morbidities (in particular frailty, COPD, and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme). Deliver patient facing/remote clinical medical reviews, including long-term conditions clinics and ensure medicines optimisation. Make appropriate recommendations to GPs, nurses and paramedics for medicine improvement and monitoring. Work with both the care home staff and PCN staff to improve the safety of medicines ordering and administration. .To offer support to the Single Point of Access team that supports all the Care Homes in the network. Attend and/or refer patients to multidisciplinary care conferences. Provide specialist expertise in the use of medicines whilst helping to address both the public and social care needs of patients at our PCNs Practice. Provide training/development to primary healthcare teams on therapeutics and medicines optimisation. Work with the practice team to ensure compliance with CQC standards.
