Job Overview
Band 7 Community Matron - 37.5 hours per week
North East London Foundation Trust has an exciting opportunity for a Community Matron in Romford Clinic, ICT Central.

Main Duties of the Job
The post holder will work as a complex case manager, without direct supervision. You will be involved in delivering integration, working with GPs, social services, and third sector organisations.
You will provide case management, facilitate early discharge from hospital, and prevent unnecessary/unplanned hospital admissions where appropriate, as well as support healthcare closer to home.
The post holder will be expected to support their team, department, and organisation to achieve the Trust’s Values in their day-to-day work.
The post holder will be a Registered Nurse with at least 2 years of community experience managing a complex caseload of patients. You will be responsible for a caseload of patients with complex health and social care needs, working autonomously to case manage these clients and using advanced nursing skills, such as assessment and prescribing to manage their conditions in the community.
They will work as part of the integrated care team, liaising with GPs, other professionals, and statutory and voluntary agencies to deliver individualised care to patients.

* Must have completed a mentoring course
