The post holder will be required to provide a comprehensive administration and service co-coordinator role to ensure the smooth and efficient running of the IDS / Care Transfer Hub (CTH) on a daily basis., This role will involve assisting the IDS Lead with the operational performance of the CTH, ensuring key performance, quality and financial targets are met and the service is developed as per Divisional plans, and in accordance with BSW Integrated Discharge strategy.In conjunction with the IDS Lead, ensure staff performance is maximised.
