We are now looking to recruit a Clinical Nurse Specialist Hospital2Home to work as part of the multi-professional team. In the first instance, you will need to be a registered nurse with at least 3 years post registration experience in cancer/palliative care, and community experience would be a bonus. Educated to at least degree level, you will also need to be confident in delivering education and training to all staff groups, having attained some form of post registration qualification in this. This post will offer this service to patients at both the Chelsea and Sutton sites of the hospital. We would welcome and encourage an informal visit.
The post holder will be expected to liaise appropriately with all key members of the multi-disciplinary team and service users related to the specialty. In addition, the post holder will be required to liaise with their peers across the Trust, the divisional management team and appropriate corporate directorates and teams. The post holder will be expected to liaise with colleagues external to the Trust and national networks as appropriate to the specialty.
