A Monday-Friday 8.30am-4.30pm working pattern is just one of the attractions this opportunity provides. We are looking to recruit a Band 6 Clinical Nurse Advisor based at Cremyll Road in Reading. With a genuine passion for improving patient care, you will be an enthusiastic and experienced registered nurse. With the drive and determination to make our team excel, you will have excellent communication and people skills, and be persuasive and motivational when collaborating with our integrated teams. Responsible for ensuring that our service delivers its optimum performance, you will work to achieve a seamless service. Driven to improve patient experience, you will be part of the triage team assessing referrals in a timely manner and signposting to other teams and services delivering care needs. Main duties of the job
* Provide clinical support, assistance, and professional expertise to all callers to the Community Nursing Triage Telephone Support. This will range from signposting the referral to the appropriate service, making a direct referral, or offering advice.
* Work in conjunction with administrative support staff to provide high quality, clinical triage using knowledge, skills, critical thinking, and professional judgement.
* Control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.
* Possess the ability to prioritise tasks, cases, and situations appropriately to ensure effective delivery of services during periods of high demand.
* Possess relevant clinical skills to be able to visit urgent visits in the patient's home, such as catheter care and end of life care.
